The Perth Amboy Vital Statistics office maintains records of Birth occurred in the municipality from 1854 until the present and most records of Marriage and Death events since 1895. It also maintains Domestic Partnership records from July 2004, and Civil Union records from February 19, 2007 until the present.
As of August 2010 the fee for a birth certificate is $15, marriage certificate is $15 and the death certificate is $10. Cash, money order or certified checks are acceptable.
